APPLICATION FOR ADMISSION — 2017-2018
Class 2021 Admissions Cycle
Thank you for your interest in applying to the Sackler School of Medicine’s New York State/American Program.
The deadline for applications and all supporting documents is 4 p.m. (EST) on May 10, 2017.
Instructions for Completing the Online Application
- Click on the link below to open a PDF of the application.
- Save the application to your desktop.
- You may complete the application at your leisure. However, be sure to save it each time you work on a portion of the application, so you do not lose previously entered information.
- Do not click on the “submit” button until you have completed the entire application.
- You will be able to attach the relevant documents after you have clicked on the submit button. If you are unable to attach one or more documents, you may mail them to Sackler School of Medicine, Committee on Admissions, 17 East 62nd Street, New York, NY 10065.
- Be sure to print out a copy of your completed application for your records before you submit it.
- When you have completed the application, click on the “submit” button.
- After you have submitted your application, you will be asked to pay a non-refundable fee of $100 via PayPal. Your application will not be processed until payment is received.
Before you click on the link to the online application, we strongly advise you to download Adobe Acrobat Reader. Non-Adobe PDF readers will not provide you with full functionality.
Instructions for Submitting an Application by Mail
If for any reason, you are unable to complete the online application and need to submit your application by mail, please contact us.
Important Instructions for MAC and/or iPad Users
You will not be able to submit a completed application from a MAC or iPad, unless you follow these steps exactly before you click on the “Start Application” button.
Apple Preview will not work for completing the application form. Please only use Google Chrome or Mozilla Firefox browsers. Certain functions of the application will not work in Safari.
- You must download and install the latest version of Adobe Acrobat Reader X. It is a free download. There is a new app for the iPad. It is Adobe Acrobat Reader and it is free as well.
- On Sackler’s website, click on “Start Application” button, while the Adobe Acrobat Reader is open on your MAC or iPad.
- Save the application form in Adobe Acrobat Reader.
Do not open the PDF by double clicking. You must open the application form from Adobe Acrobat Reader.
That’s all that you need to do. We apologize for this. If you are still having issues let us know, and we will then give you permission to mail in the application.
